Mutation error

Currently, there seem to be some duplicates in the mut field of the BirthDeathModel class after the simulation is performed. The problem leads to incorrect output tsv with mutations and, afaik, the reason is somewhere in the base code, not the output interface.

I'll investigate that in detail soon.

Confirmed - the duplicate mutations have different times, so they must be generated by different AddMutation function calls. Therefore, it is a problem of event generation, not simple output mistake
