Description
In the sign up email below, the sentences circled in red are not completed properly and Should MammoChat or Mammogram be constant throughout email?
On consent page, can the PI and Co-PI be on separate lines and not jumbled in the title. Same for the rest of the consent document.
How are participants going to add signature? Upload what file?
After completing consent, website does not take participants to survey questions or authorization form right away. It takes them to their dashboard. Some participants may leave website thinking that everything is completed, not knowing to open the other required forms/ questions. Coud all forms follow the other so that we get the information needed and not having to call patients to complete everything and having to tell them how to do so?

My study dashboard signs participant out of account and back to log in page.
Was not able to complete the rest of questionnaire (questions were not visible/ did not see a “next” icon). Questions stopped at #9. Unable to go back in and complete.
Emails sent to participants email address may be lost in inbox. Can the name change from “no-reply”?
