Issue Description

A thorough description of the problem, including:
I somehow obtained a strange item that told me to come report it here, it looks like a brown, redish fish, but instead of a face it has a glowing yellow hole
Mod list: i have a lot of mods, relevant one here tho would be random Terraria Random Loot as it gives ennemies a chance to drop anything as random additional loot
Reproduction: i couldn't, as i expect it was an extra drop from Terraria Random Loot, and reproducing it would take a long time of killing a lot of ennemies and a lot of luck to get that one specific item again
