[feature] Improve monthly PDF report

Now it has incoming and fee values shown in fiat.

1) For accounting purposes same values also in bitcoin would be great help.

2) To complete perfecting, would be nice to see BTC rate in fiat, calculated from BTC amount after fees. That would represent price on which merchant has "bougth" the amount of bitcoin show on 1).
