Simple agent chat example is returning duplicates

examples/simple_agent.qtype.yaml is returning duplicate responses

I reproduced this with the same prompt.
This isn't returning duplicates -- the react agent is stuck in a loop because it thinks it can't answer the question with the tools it has.

Sometimes it runs for a while and returns an error about meeting "Max iterations of 20 reached! Either something went wrong, or you can increase the max iterations with .run(.., max_iterations=...)"

I tried adjusting the system prompt but got the same results.

Ultimately this is due to how llama index implements the ReAct agent. I don't love their implementation and plan to switch off of it, but for v1 this is working as expected :(

Leaving the issue open since it reflects a good example of why to move off llama index's ReAct agent
