Team updates not properly handled

Right now, dissolved teams correctly delete submissions. However there are other mishandled scenarios:

• An existing competitor starting a team and resubmitting currently result in the submission list showing just the existing competitors' submission
• A new competitor starting a team with an existing competitor currently result in the submission list showing a submission from the new team separate from the existing competitors' submission

In reality, what should happen is:

• Creating a team, adding members to the team, removing members from a team, changing the team name, and deleting the team should all update the submission list, the submission indices, and in turn who is attached to which teams
