Documentation bug: Examples should show explicit disconnection of the client

With the change to requiring a Client supervisor for your apps supervision tree, it is very possible to start to leak processes over time if they are not explicitly closed. I had, incorrectly, relied on the garbage collector to clean up connections when they went out of scope in the past and that is now explicitly not correct and a potential problem waiting to happen.

This seems to be a pre-existing bug in Supervisor. How are you using the library? Do you have any examples we can use to reproduce the error? Could you give us more information?
