Add Issue and Feature Request Templates

Currently, there is no template provided when raising an issue or feature request. As a result, it can be challenging to fully understand the problem based solely on the title. Providing more context and detailed steps to reproduce the issue would greatly enhance our ability to comprehend and address the problem effectively.
