Items do not stay equipped after dying

V1.2.7 Tested mod on its own with SPT 3.10 to make sure it wasn't an issue with a conflicting mod
Insured items both through right clicking and going through the pre raid insurance screen
After dying in raid items are lost

That's odd, try again with v1.2.8 and see if the same behavior occurs. Like Purple said, are you using any mods that others have had issues using as well?
