Biomedical Concepts generator needs to be fixed

Biomedical Concepts generator needs to be fixed after the introduction of the new biomedical_concept, biomedical_concept_property, alias_code and code tables.

The majority of attribute values can be found in these tables.

Keep the API helpers. These will be needed for populating the synonyms for the biomedical concepts and responseCodes for the biomedical concept properties (DSS).

Need to assess which values are held in the database and prefer these values to those accessible through the API.
