Certain misuses can cause misleading errors

There are a couple misuses of the application that can cause confusing/misleading errors. Need to throw more explicit/detailed error message to make clear how the library is being misused.

Scenarios are:
- Trying to change nodes from different documents in the same transaction
- Performing an operation that results in a node having multiple attributes with the same name
