Category: The product category of the devices. Drop down selection. See Categories section below.
Brand: Drop down selection of pre-specified brands, with freetext option.
Model: Freetext.
Age: Estimated age of the device in years.
Repair status: the outcome of the repair attempt at the event.

  - Spare parts required?: Yes/No
- Repairable:
  - Next steps: More time needed / Professional help / Do it yourself
  - Spare parts required?: Yes/No
- End of life:
  - Barriers to repair?: Spare parts not available / Spare parts too expensive / No way to open the product / Repair information not available / Lack of equipment
Description of problem/solution: Freetext.
URL of useful repair information used: Links to useful resources for future repair attempts.
Source of repair information: From Manufacturer / From 3rd Party
Suitable for wiki: Yes/No. Could the solution comments help Restarters working on a similar device in future? Or is it a fun case study?
Add image: Image upload of the repair attempt in process.
Weight: If category is 'None of the above' (i.e. Misc), then weight of the device is recorded. The weight is used for the environmental impact statistics.
