Support for non-default pg_port

The configured pg_port was not supplied to the psql -c commands, thus all commands were executed against the default port of 5432.

I can confirm this is still an issue. I've had to update my Gemfile to:

gem "capistrano-postgresql", require: false, github: "skillstream/capistrano-postgresql"

to ensure the commands are run against the non-default port I have set in the deploy.rb file:

set :database_port, 5413
set :pg_port, -> { fetch :database_port }
