This adds a few indexes that were missing; without them, I was seeing huge numbers of full table scans.

Specifically, on stableGeneIds.hgnc_symbol, LRG_proteins.RefSeqProteinID, and LRG_transcripts.RefSeqTranscriptID.


          Add more indexes
